In order to maintain the quality of our vendor list, we are asking that you complete and return to us the attached VENDOR QUALIFICATION FORM.  It is important that all information requested be provided so we make informed decisions regarding each vendor.  Failure to submit all information will delay your opportunity to do business with the District.
